Company background:
American Longevity Healthcare, Inc., (ALHC), is a California international corporation established in September, 2002. ALHC was founded by group American professional health care providers who are medical doctors and gerontologists.

ALHC is the first company in U.S.A. that provides a unique internationally, bilingual and professional gerontological consulting services for the programs and projects that are focused on the long-term care services (health care and social services) for the elderly either in America or in Mainland China.

ALHC is an experienced group of "networked" professionals from medical geriatric and social service backgrounds and disciplines providing development and management consulting services for sponsors of Adult Day Health Care (ADHC), Adult Day Care (ADC), Home Care, Residential care, Retirement community services, and Assisted Living projects throughout the United States and mainland China.

Our team of professionals at American Longevity Healthcare, Inc., includes: Western medicine, geriatric medicine, Chinese Traditional Herb Medicine and Therapy, and health care and social services for the elderly. The team, which includes professionals with experience in development, administration, management, operations, marketing, human resource management, and budget control, has worked in the health and long-term care industry for the elderly in China as well in America for many years. Because of the founders’ extensive knowledge and experiences in the administration and operation in the filed of clinic medicine and geriatric health care, the company’s services are especially developed to meet most needs of people who are either suffering from chronic health problems and/or looking for a higher quality of the life, and a healthy longevity.

Services Provided for Elderly Long-term Care Projects:
Program and Service Development and Improvement: complete development services for ADC, ADHC, home care, board-care, senior housing, retirement communities, and assisted living facilities (includes new construction, expansion, remodeling and conversions). Program marketing and financial feasibility, site selection, program design which includes the human resources and services, budget planning, licensing, and start up services, also to improve the current services.
Program Management: provide the professional contract management of ADC, ADHC, home care, board-care, senior housing, retirement communities and assisted living facilities, for both existing and new start up facilities.
Program Operations: management consulting for ADC, ADHC, home care, board-care, senior housing, retirement communities, assisted living, and skilled nursing facilities, with emphasis on performance improvement, turn around of under performing facilities, new program and service development and planning.
Program assessment and evaluation: provide a professional assessment and evaluation services, to find out the right resolution for the targeted problems.
Service follow up and quality control: provide the follow up service to make sure a high quality of service and documentation for an ongoing operation and management of the project stated above.

The Goals of Service for Adult Day Health Care Center ( ADHC) in California:
To improve the quality of comprehensive health services and increases the quantity of the services in the local community for the frail elderly who are struggling in the chronic health problems and multiple disabilities.
To comply with Title 22 regulation of California Department of Aging, and the service license standards required by the Department of Health Services and Medi-Cal Office.
To continually qualify the State licensing requirement for ADHC program and pass State annual survey or license renewing each year.
To extend the ADA to their maximum capacity approved by State by developing a market plan and strategy.
To have a successful achievement either for the services or for the benefits and profits of the business. This can be achieved by a quality control of the services and budget control of the business.
The Specific Services Provided for ADHC in California:
A. Program assessment and evaluation
Program needs assessment and evaluation (human resources, staff arrangement, cost efficiency, current deficiencies and problems in the program operation and management).
Analysis the problems of the operation which happened often in most of ADHC.
Analysis the problems and the deficiencies, and conduct the strategies and plans for the solutions.

B. Improvement of Program Management
Establish the program data base and management system for participants admitted.
Establish the management system for medical billing process and follow up.
ADHC Multidisciplinary Team building and human resources, Department, Personnel, and Responsibilities, (staff recruitment and replacement, and staff ratio).
Provide the strategies and plan for the management of human resources without the deficiencies (personal files, update information, and follow up).
Provide the consultation to conduct the company’s personal policy for a better professional personnel management and effective work environment.
Provide the consultation to establish the professional procedure of Utilization Review demanded by the Sate for the quality control of the program.
Have a professional management for the Charting and Filing procedures.
Provide the training and consultation for MDT members, how to conduct a professional needs assessment which includes the initial and reassessment/quarterly too, also, the tools or resources.
Provide the training and consultation for MDT members the Assessments and IPC Writing, how to write a professional IPC (RN, SW, LCSW, OT, PT, ACT, and RD), which includes the coordination of the IPC among the MDT members.
Provide the training and consultation for MDT members a computerized IPC networking system. It is more convenient for MDT members’ professional documentation in term of the quality control and writing efficiency.
Provide the training and consultation for MDT members, how to conduct a professional service flow sheet without the deficiencies.

C. Improvement of Program and Service
Provide the training for MDT members, how to conduct a professional Participant Intake and Admission procedure and process without the deficiencies.
Provide the consultation and training to prepare a better TAR and IPC and reduce the numbers of the denied TAR or IPC either for the initial or for the reassessment.
Provide the consultation and training to conduct a professional home visit and evaluation during the in-take process.
Program team training (MDT team and Program Aides) for understanding ADHC services, the role of team playing, and the professional coordination for the program.
Provide the specific professional training for RN department, SW department, OT/PT department, RD, and Activity department by a different need or deficiency (quality of the services, quality and organization of the documentation, and work efficiency).
Will provide the professional services for the company’s Correction Plan after the State annual survey if needed.

D. Program Operation
Provide the strategies and plan for Program Daily Operation and Management, and the management of MDT team & program aides.
Provide the training and consultation for MDT members, how to follow up your IPC to implement your services for participants without the deficiencies.
Provide the training and consultation for MDT members, how to conduct your service progress notes without the deficiencies
Provide the training and consultation for the management of daily attendance and billing log without the deficiencies.
Provide the training and consultation for the management of transportation services without the deficiencies.
Provide the training and consultation for the nutritional services without the deficiencies.
Provide the training and consultation for the management of participants which includes the attendance and follow up.
Provide the training and consultation for ACT to design the meaningful daily activities which includes the designed activity plan and the resources provided (staff and programs). It is very important to develop your business market and attract more elderly coming to your program or maintain in your program.

E. Service follow up and quality control
A follow up service will be provided for ADHC on a regular basis and make sure that the MDT members and staff of ADHC perform continually the professional services and documentations. It will be suggested for any further correction if the deficiencies were found again.
To have a quality control for the program operation and documentation. It could be possible to suggest the company for staff replacement and recruitment if needed.
The follow up and quality control will be ongoing basis for the program operation.

The Term and Cost of the Services:
The professional consulting services will be started under a signed contract of the professional service between ADHC and American Longevity Healthcare, Inc., At least, one year contract signed will be required to implement the services stated above. However, any party can cancel the contract at any time if needed without any condition. One month notice is given to other party will be appreciate if it is possible.

The company will charge for the services either for monthly or for hourly rate. The paid services are included in the fine professional services designed and the processes planed well, the special professional knowledge and the comprehensive experiences in this business, the great responsibilities for the business license and continual operation, the time and transportation cost. And, more valuable and important is the guaranteed outcome for an ADHC State license & certificate , improvement and growing of the program, and a higher business net-income than before.
The Flexible Service Schedule
We provide a flexible service schedule for your business, which it could be in the weekend.
We offer a telephone communication and consulting services for your MDT members if needed. Also, it could be a follow up and monitoring process for the services provided.
